Clarence Town Healthcare provides general practice services to support the health needs of older adults in community and residential aged care settings within NSW. Care is delivered by independent doctors and focuses on appropriate medical assessment, continuity of care, and coordination with nursing staff, allied health providers, and carers where relevant. The aim of these services is to support the health, comfort, and dignity of older patients through professional and ethical medical care.

Medication Management Reviews

  • Medication management reviews may be arranged to assess current medicines and identify any issues related to use, effectiveness, or safety.
  • Where appropriate, reviews are coordinated with the patient’s usual GP and a participating pharmacist, and may be conducted in the patient’s home or care setting.

Health Assessments for People Aged 75 Years and Over

  • Patients aged 75 years and over may be eligible for an annual health assessment conducted by an independent doctor.
  • The assessment allows time to discuss health concerns, daily functioning, and ongoing care needs, with appropriate advice, monitoring, or referrals provided based on individual circumstances.

Health Assessments for Residents of Residential Aged Care Facilities

  • Health assessments for residents of aged care facilities may be undertaken to review overall health and functional status.
  • These assessments may include consideration of mobility, cognition, pain, skin health, continence, hearing, vision, and oral health, with the aim of supporting appropriate care planning and ongoing management.

Coordinated Veterans’ Care (CVC) Program

  • Eligible veterans may be supported through the Coordinated Veterans' Care Program, where applicable.
  • This program involves collaboration between the patient's usual GP, other healthcare providers, and care coordinators to assist with the ongoing management of chronic health conditions, regular review, and coordination of care.
