Chronic Disease Management

Over half of all Australians live with two or more risk factors for chronic diseases, especially those aged between 45 and 64. These conditions often impact work, daily life, and long-term health. At Clarence Town Healthcare, our independent doctors focus on early detection and consistent care to help manage ongoing health issues.

During your visit, our GPs can create a tailored GP Management Plan (GPMP) and, where needed, a Team Care Arrangement (TCA). These plans support care that is aligned with your individual needs and may also include access to Medicare-supported allied health services.

If you meet the eligibility criteria, our doctors will arrange a time to assist you in developing your GPMP and TCA, including the necessary Allied Health referrals. The appointment typically takes around 30 minutes, though this may vary depending on individual circumstances. Referrals to Allied Health professionals are arranged as part of the care planning process.

The Key to Better Health

GPMP and TCA consultations are bulk billed for eligible patients with a valid Medicare card, in accordance with Medicare requirements. If you wish to support the management of your chronic condition, your GP can discuss appropriate care planning options with you.
