Over half of all Australians have at least two risk factors for chronic diseases, which affect more people and keep them away from work. Half of Australians aged 45–64 have one or more chronic diseases, making Chronic Disease Management an essential concern for the Clarence Town Healthcare Independent Doctors. During consultations with our independent medical practitioners, you can consider tailoring your GP Management Plan (GPMP) and Team Care Arrangement (TCA) to suit your needs and the preferences of your loved ones.

A GP Management Plan
- A documented care plan designed specifically for chronic disease management is created after a visit with our independent GP. During the creation of a chronic disease management plan by a GP, your medical, physical, psychological, and social needs are all taken into account. After implementing a GP Management Plan, it should be reviewed every six months or more frequently if your situation changes significantly. Clarence Town Healthcare will notify you when your plan is about to expire or needs to be renewed.
A Team Care Arrangement
- A Team Care Arrangement can be made along with a GP care plan. A Team Care Arrangement is similar to a GP Management Plan; it helps you get healthier by focusing on the reasons for any long-term conditions you may have. Through a Team Care Arrangement, Medicare will cover Allied Health visits for patients needing care. This has been scientifically shown to improve your health and happiness.

If you meet the criteria, our doctors will arrange a time to assist you in developing your GPMP and TCA, including the necessary Allied Health Referral. During this time, we will conduct an in-depth medical examination, collect vital information, and coordinate our efforts with those of any other treating medical professionals. The entire procedure takes no more than 30 minutes, and you’ll quickly receive your referral to an Allied Health professional. Expect a minor delay when direct contact with other healthcare specialists is impossible, but be assured that we will expedite the procedure as much as possible.
Patients who qualify will be pleased to know that their sessions with our doctors to complete the GPMP and TCA will be free. Access is further expanded because no payments are required with a valid Medicare card.
The Key to Better Health
You can receive up to five subsidised Allied Health consultations per year, and a new plan becomes available to you when a year has passed.
