Our Aged Care Specialist Medical Service focuses on delivering regular, high-quality care to older adults across both community and residential aged care settings in NSW. We aim to provide efficient and responsive services while supporting nursing staff in delivering consistent care for residents.
Clarence Town Healthcare’s team of independent specialist doctors works closely with other medical professionals and allied health staff to ensure each patient receives dedicated, coordinated care. Our commitment is to support the health and dignity of elderly patients and their carers within the local community.
Medication Management Reviews (approx. 60 minutes)
- Our doctors aim to optimise your drug treatment by identifying and resolving any associated issues, ensuring you receive the maximum benefit from it.
- Your primary care doctor (PCP) will set up a review at your home with a nearby pharmacist.
Health Check for people aged 75 and older (annual) (approx. 60 – 90 minutes)
- If you are 75 or older, you should use our annual Health Checkup that is made just for you.
- In only 60 to 90 minutes, you can talk openly with our independent, trained doctors about any problems and work with them to find answers that will improve your health and quality of life.
Health Check for Residents of Residential Aged Care Facilities
- Our aged health checks are designed by our doctors to identify avoidable health issues so elders can receive appropriate care.
- Our doctors evaluate individuals' daily functioning, cognitive abilities, pain management, foot care, skin integrity, continence, hearing, vision, and dental health to enhance their quality of life.
Coordinated Veterans' Care (CVC) Program (approx. 60 – 90 minutes)
- The CVC Programme helps Gold Card holders with chronic conditions escape unnecessary hospitalisations by providing professional help.
- Our doctors collaborate with your PCP and NP to assist in managing your condition, enhancing care coordination, and ensuring regular check-ups. You will receive a personalised care plan tailored to your needs, enabling you to monitor your treatments, follow-up appointments, and achieve your overall health goals.
