Chronic Disease Management
Over half of all Australians live with two or more risk factors for chronic diseases, especially those aged between 45 and 64. These conditions often impact work, daily life, and long-term health. At Clarence Town Healthcare, our independent doctors focus on early detection and consistent care to help manage ongoing health issues.
During your visit, our GPs can create a tailored GP Management Plan (GPMP) and, where needed, a Team Care Arrangement (TCA). These plans ensure your care aligns with your individual needs and may also include access to Medicare-supported allied health services.

A GP Management Plan
- A documented care plan designed specifically for chronic disease management is created after a visit with our independent GP. During the creation of a chronic disease management plan by a GP, your medical, physical, psychological, and social needs are all taken into account. After implementing a GP Management Plan, it should be reviewed every six months or more frequently if your situation changes significantly. Clarence Town Healthcare will notify you when your plan is about to expire or needs to be renewed.
A Team Care Arrangement
- A Team Care Arrangement can be made along with a GP care plan. A Team Care Arrangement is similar to a GP Management Plan; it helps you get healthier by focusing on the reasons for any long-term conditions you may have. Through a Team Care Arrangement, Medicare will cover Allied Health visits for patients needing care. This approach supports coordinated care for people with long-term health conditions.

If you meet the criteria, our doctors will arrange a time to assist you in developing your GPMP and TCA, including the necessary Allied Health Referral. During this time, we will conduct an in-depth medical examination, collect vital information, and coordinate our efforts with those of any other treating medical professionals.The appointment typically takes around 30 minutes, and referrals to Allied Health professionals are arranged as part of the care planning process.
The Key to Better Health
You can receive up to five subsidised Allied Health consultations per year, and a new plan becomes available to you when a year has passed.
